As from 25th of May 2018, I am required by law to provide the following information to you regarding how your personal data is collected and processed.
Who I am: The Encouraged Self CBT Talking Therapy is operated by Lead Therapist Sheena Ruparelia. You can contact the Encouraged Self CBT on e-mail: enquiries@theencouragedselfcbt.co.uk.
How data is collected: This includes data given by you such as personal information and data given automatically which can be from the booking system. Only what is required and necessary will be collected. Occasionally, I might receive your contact information from a third-party referral source or partner organisation. If we do, we protect it in exactly the same way as if you give it to us directly. you will be required to complete the Therapy Agreement that covers further personal information gathered about you and your agreement/non-agreement to it’s being shared with relevant parties only. Relevant parties can include your GP or clinically responsible professional e.g. your psychiatrist, your services funding party e.g. health insurance, employer etc.
Keeping Data Secure: All data collected is held in a secure place.
Data Retention: Data is held as long as necessary and required. All data is kept secure via a protected system that belongs only to Sheena Ruparelia. Please contact me to discuss this further if you would like more information on how your data is stored.
Your rights
To be Informed: A Therapy Agreement will be given to you to sign which also states your rights.
Access: you have the right to request for copies of your data and information that The Encouraged Self CBT holds (this incurs a charge of £10). It will be aimed to be sent to you within 25 working days.
Correct: you have a right to request changes in information that the company holds that may become inaccurate. This is to ensure appropriate changes or updates can be made.
Erase: you have a right to ask for your data to be deleted or erased from the system.
Restrict Processing of Data: you have a right to limit the way your data is used or “block” The Encouraged Self CBT from using your data. However, if there is a reason to disclose information i.e. if clinical judgement suggest there is a risk of harm towards someone or children or you are at risk then other services (including your GP) may have to be contacted. In rare cases information may have to be disclosed without consent to ensure safety of individuals or unless there is law enforcement involved.
Data Portability: you have a right to request that we move/copy/transfer your data.
Object: The right to object use of your data. However, if there is a reason to disclose information i.e. if clinical judgement suggest there is a risk of harm towards someone, towards children or you are at risk then other services (including your GP) will be contacted. In rare cases information may have to be disclosed without consent to ensure safety of individuals or unless there is law enforcement involved.

Complaints: You have a right to complain to the Information Commissioner’s Office (ICO) www.ico.org if you have any concerns about use of your data.

General: you may not transfer rights to any other person.
Changes to Policy: From time to time the policy will be updated (as required by law). The Encouraged Self CBT reserves the right to make changes. Any updates can be checked by seeing the last reviewed date.
